What is the Family Health Index™ (FHI)?

Cleo’s Family Health Index™ (FHI) is a proprietary, data-driven tool that measures caregiver and family health across multiple dimensions. It’s the only advanced analytic model of its kind that proactively identifies members at higher risk of burnout or in need of timely intervention — before issues escalate.

By analyzing physical, mental, social, and financial health indicators, the FHI provides employers with actionable insights to better support their populations. These insights help guide targeted interventions, improve health outcomes, and reduce the downstream impact on productivity, retention, and healthcare spend.

How is the FHI measured?

Caregiving often leads to significant challenges, including chronic stress, emotional strain, physical health issues, social isolation, and work-life imbalance. To address these complex realities, the FHI measures seven key dimensions of family health: Confidence, Caregiving Balance, Self-Care, General Health, Mental Health, Connectedness, and Family Support.

Members are invited to complete the FHI at enrollment and every six months thereafter. Through a short in-app assessment, each member receives a score for each of the seven dimensions and an overall score that reflects their risk level — from low to moderate (monitoring) to high (nurturing).

Since launching in 2023, over 12,500 Cleo members have completed the FHI. Members identified as moderate or high risk receive personalized in-app Support Plans, curated resources, and guidance from a dedicated Cleo Guide, tailored to their caregiving journey.

The FHI gives employers and health plans real-time visibility into population-level caregiver risk — allowing for timely, targeted interventions that drive measurable outcomes in health, productivity, and retention.
